The CPTS offers the check-up
An area-wide campaign or invitations from members: doctors’ patients, pharmacies, nurses. Each practice shares it with its own link.
The patient prepares at home
A smart questionnaire on a smartphone, in just a few minutes. Risk factors are identified before the consultation.
The member doctor leads the consultation
The check-up arrives structured: the doctor reviews, adjusts and signs the Personalized Prevention Plan (PPP) — a consultation registered under the “Mon bilan prévention” program.
The CPTS manages its area
Check-ups completed, risk profiles, referrals made: aggregated, anonymized indicators, ready for your ACI reports (the ACI, accord conventionnel interprofessionnel, is the agreement with French National Health Insurance that funds CPTSs).
