Patient questionnaire
Using the individual link sent for their appointment, the patient confirms their identity and then fills out the Sokrate 360 questionnaire on a smartphone or computer. The questions adapt to their profile and answers: medical history, lifestyle habits, sleep, psychological well-being and other areas relevant to prevention.
Answers are saved as the patient goes, so they can pick up where they left off after an interruption. Closing the page does not erase them. The patient can review their answers and separately choose their preferences for referrals and prevention messages.
During and after the questionnaire
AI analysis & structuring
Depending on the questionnaire, AI assistance may suggest clarifying questions while the patient is filling it out. Once the answers are collected, it helps structure them, prepare a summary and highlight points to discuss with the healthcare professional.
Automatically generated summaries and suggestions are preparatory documents. They may contain errors and must be reviewed by the professional before being used for any medical decision.

Medical validation
The healthcare professional reviews the answers and the preparatory documents, can add to or correct them, and then sets the patient’s priorities and Personalized Prevention Plan (PPP) together with them. Generating a document and validating it medically are two separate steps.
When tele-expertise is offered as part of the pathway, a clinical question is first reviewed and validated before being sent to a specialist. The specialist gives their opinion to the requesting professional; exchanges and responses are logged.
End of the consultation
Personalized plan delivered
The professional explains the agreed actions and follow-up arrangements to the patient. Depending on the pathway, the documents are available in the patient’s personal account and made available to the team caring for them, including in the center’s appointment booking tool.
A preparatory document available before the consultation does not replace the plan validated with the professional. The referrals suggested take into account the needs identified and the center’s resources, along with the sharing choices set up for the pathway.
According to the agreed pathway
Long-term follow-up
With the necessary preferences in place, the patient can find their referrals and useful links for booking appointments. Messages and reminders may accompany the next steps. The coordination team tracks the actions and can help the patient when a step is still pending.
Clicks on referral links, the patient’s choices and follow-up actions may be recorded to organize this support. A click does not prove that a consultation took place. The patient can change their preferences and exercise their rights over their data.

In the app, they find the same medical voice when it’s time to understand their results and take action.

Documents for the professional
A summary prepares the consultation. The professional reviews the suggestions, adjusts them and sets the actions with the patient.
Targeted referral
Depending on needs and available resources, the pathway suggests a referral to an appropriate professional. Tele-expertise can supplement the doctor’s opinion when this service is offered.
Tracking next steps
The coordination team tracks steps and exchanges, identifies pending actions and can help the patient overcome any difficulties they encounter.
