Terms of Use — Patients

1. Your pathway

• Access to the pathway: depending on the service used, you access the questionnaire from your account or following an invitation from your healthcare professional or health center.
• Questionnaire: you enter your responses in the dedicated area. Their transmission to the service is encrypted.
• Personal account: account creation and login procedures are specified in the area being used. Keep your login credentials and access links confidential.
• Preparatory documents: summaries may be generated automatically from your responses. They prepare your discussion with a professional and do not, on their own, constitute a validated Personalized Prevention Plan (PPP).
• Consultation and plan: as part of a check-up with a healthcare professional, the professional assesses your situation and adjusts and validates your PPP. An automatically generated summary does not constitute validation or signature by the professional.
• Sharing: the recipients and sharing arrangements, including with your regular doctor (médecin traitant), are described in the privacy policy.
• Follow-up: support is provided by a medical coordinator; once this feature is activated, you will be able to link connected devices (activity and health measurements) — an optional link that you can revoke.

2. Your commitments

• provide accurate and truthful responses — inaccurate responses may distort the structured file provided to the healthcare professional;
• keep your login credentials confidential; any activity carried out from your account is deemed to be yours;
• reserve clinical information for the health areas designed to collect it; do not send results, symptoms, or medical history through a general contact form or by ordinary email;
• use the service in accordance with its intended purpose and not compromise its security or proper operation.

3. Free of charge

The prevention check-up falls under the official Mon bilan prévention program, France’s national prevention check-up program, and is covered by French National Health Insurance (Assurance Maladie) under the conditions in force. Use of the service is free for the patient.

4. Your data and your consents

The processing of your data is described in the Privacy Policy. Certain uses are based on separate, optional consents that you may withdraw at any time: participation in prevention initiatives within the partner center, sharing of your results with the relevant healthcare professionals at the center, referral to partner professionals, and targeted medical communications. You may also object to the referral letter being sent to your regular doctor and to the use of your pseudonymized data for research. In all cases, you retain complete freedom over the use of your results, which you can export to share with the healthcare professional of your choice.

5. Deleting your account

You may request deletion of your account by writing to dpo@sokrate.com, without attaching any clinical data. You will receive a response within one month. This period may be extended by two months due to the complexity or number of requests; you will be informed of any extension, and the reasons for it, within the first month.

Closing your account and retaining your medical record are governed by separate rules. Certain data remain necessary for the healthcare professional to meet their record-retention obligations. The applicable criteria are set out in the Retention periods section of the privacy policy.

6. Nature of the service

Sokrate does not make any diagnosis or issue any prescription. The information provided is not a substitute for a medical consultation. Clinical decisions rest exclusively with the healthcare professional, who is the author and signatory of the PPP. For all other matters (intellectual property, liability, governing law), the Terms of Service apply.
